Arranging Care

You will meet with a Dobson Nurse Care Coordinator to discuss what you are looking for, what Dobson can help you with, and develop a plan together. Once a plan has been completed, Dobson Healthcare will match your needs and wishes with the appropriate caregiver.

Please keep in mind that your Nurse Care Coordinator will visit you periodically to see how your services are going. Also, you have access to your Care Coordination Team to discuss any changes you would like to see.

If you or your family member is wanting services….

Please call us at 866-866-8984 to speak to an intake nurse. They will ask you a series of questions that will help determine what you are looking for and how we can help. At that time, together we will talk about a good time for our Nurse Care Coordinator to come meet with you and your family to discuss the details. There is no obligation to accept services with Dobson and the meeting is free of charge to you. If you are a case manager, discharge planner, or healthcare provider….

Please call our intake nurse at 866-866-8984 and inform them of what you are looking for. It is helpful if you have the following information available:

• Client Diagnosis

• Location of the Client

• Services needed and type of care provider

• Tentative date services will start

• Payer Source

Once you and the Nurse Care Coordinator have discussed  the above, we will then discuss what is best in terms of setting up a time and place for meeting with you, the client, involved healthcare providers, and the family. Many times we will set up several meetings depending on the acuity of care needed and case manager’s preference.

If the client is in a facility, we will meet you and the client there to discuss the best solution for a transition home. Dobson will work with you, the client, facility and our Discharge Team to help coming home as smooth as possible. Please note that you there is no obligation for you or your client to accept services through Dobson Healthcare.

Services and the associated rates vary based on the level of care and caregiver you will need, the services you are requesting, and how often you will need services. Please note that include in all rates is access to a Dobson RN Nurse Care Coordinator 24 hours a day/7 days a week and also Care Coordination as you need it.
