Care Coordination Team

Continuity of Care is more than a “coined” healthcare term to Dobson; it is an expectation. A balanced management team (including scheduling, nurse care coordinators, & caregivers) and a state of the art client care management program allows Dobson to better identify the best caregiver match for you and/or your clients. The Result. Consistent care and communication with everyone involved.

Your Care Coordination Team is available 24 hours, 7 days a week for any updates and concerns they may arise. Your Nurse Care Coordinator and Scheduling Coordinator work with you, your family, and healthcare providers to insure you receive all care and services available to you.

Acute Care

Unforeseen circumstances change our life plans when we least expect it. Limited options are available to clients who have or will experience a need for acute care. Clients with acute care needs receive caregivers who have extensive experience with tracheotomies, ventilators, feeding tubes, traumatic brain injuries, spinal cord injuries, muscular and neurological disorders.  The caregivers may be high tech home health aides, RNs, LPNs, PARAs or a combination of all of them depending on your level of care.

Dobson understands that infusion and injection therapies are very intricate and specialized. As a result, you and your clients should expect nothing less than professional, confident, and experienced specialty nurses. Whether you are receiving a one-time or ongoing visits, you will find a distinction with our Registered Nurses. We are available to assist with IV and Injection Teachings as well as ongoing IV therapy visits. Please note that our Specialty Services must be pre-authorized through your physician and we must coordinate directly with your pharmacy.

Eldercare

Our elderly clients and their families reap the benefits of our high-tech experience and history!

Everyone knows that home care for the elderly is one of the fastest growing healthcare segments in the world. With the experience and standards Dobson requires of itself and staff, you and your family have access to some of the most qualified and seasoned caregivers in the industry! You can expect to receive the following from your Care Coordination Team:

Care Coordination (included in your services): Nurse Care Coordinators and Scheduling Coordinators are involved with your services, your care, and they maintain working relationships with your healthcare providers to ensure continuity of care on an ongoing basis.

Experienced Staff: Your caregivers offer a sense of freedom and independence which otherwise might be lost in a nursing home or assisted living. We can provide care that will complement your healthcare services you are currently receiving and if you will be coming home from a stay in a facility Dobson’s Care Coordination Team will be happy to assist with a smooth transition home for you and your family.

Dobson expects its staff to have at least 1 year of current direct client care in the healthcare industry before we will consider hiring them. Additionally, Dobson conducts competency testing, multiple interviews, drug testing, background checks, TB Testing with all staff.

Facility to Home Discharge Program

Coming home after a long stay at a hospital, rehab facility, or a nursing home always seems to pose the most stress for both the client and the family. Since Dobson Home Care will be the primary healthcare providers in your home, we have designed a team to assist your with the following aspects of your discharge to help make coming home as smooth as possible.

  1. Meet with you, your family, discharge planner, and other healthcare providers in the facility prior to you coming home to help answer any questions you may have about our services and to allow our Care Coordination Team a better understanding of the care you will need.
    • • This allows our Team to begin the process of identifying and coordinating your services prior to you coming home so you and us are prepared
  2. Work directly with your discharge planner for details on home care service needs such as:
    • • Service needed through Dobson
    • • Equipment needs in the home
    • • Coordination of Therapies (Dobson will work with your in-home therapists to ensure our services and care match your rehabilitation needs)
    • • Assist with other additional  requests from you, your discharge planner, and case manager as necessary and within our capabilities
  3. Meet you, your family, and involved healthcare providers at your home upon discharge to complete all necessary paperwork, answering other questions you may have, and for our are Nurse Care Coordinators to become acquainted with your home and routine. This allows you and the Nurse Care Coordinator to develop a plan of care that will meet your needs

  4. Ongoing follow-up and communication with you, your family, involved healthcare providers, and our caregivers to insure that your care and services continue to meet your needs.

Aide for the Day! (1x only transportation program)

Dobson offers a 1x Transportation Services for clients who have outpatient procedures or appointments that require them to have a driver bring them to the appointment, stay with them, and insure they go home.
